2011-12 Review of Accommodations Used During Testing Forms

- Form to be used during team decisions prior to testing to record required testing accommodations documented on the student's IEP, Section 504 Plan, LEP documentation, or transitory impairment documentation, for data entry into CECAS or NC WISE, and during/after testing to record the accommodations provided and used during testing.

Test Accommodations Charts
The following charts are available to LEAs for use in documenting students' required testing accommodations in Section 504 Plans, Limited English Proficient (LEP) plans/documentation, and transitory impairment documentation.
Use of these charts is strongly encouraged by the NCDPI. The charts are modeled after the IEP DEC 4 (6 of 10) North Carolina Testing Program accommodations chart, and will therefore facilitate the consistent documentation of required testing accommodations for students across plan types. The DEC 4 IEP form is available at http://www.ncpublicschools.org/ec/policy/forms/statewide/.
